D.O. students whose schools are participating in VSAS in 2012-13 may submit applications through VSAS beginning on March 1, 2012.
Canadian and D.O. students who are from COCA accredited schools that are NOT participating in VSAS may print out the application in the link below and submit it with the requisite supplementary materials:

Applications for the 2012-2013 academic year will be accepted beginning March 1, 2012. Scheduling will begin in May 2012.

- Verified Credentials National Criminal Background Check: instructions for completing the online application.
- Self Disclosure, Consent and Release of Information Form: Form can be downloaded from the above 2012-113 Additional Application Requirements link. Upload signed form to VSAS. Electronic signatures are not accepted.
- Personal Health Insurance: Mail in a copy of your health insurance card with your application.
- Immunization form: Students must submit the UW SOM immunization form included in the application packet. We do not accept immunization forms from your home school. All required dates must be entered and the form signed by your school health official or by your primary care physician. We do not accept immunization records.
- UW SOM Essential Requirements for Medical Education Form: Sign and mail in form with your application.
- UW Privacy, Confidentiality, and Information Security Agreement Form: Sign and mail in form with your application.
- Evaluation Form: Enclose an evaluation form from your school. The department will fill it out and return it to your school. Credit for the elective is granted by your home institution, not by the University of Washington.
- Copy of BLS or CPR card: If your school cannot verify your BLS on the application, you must send in a copy of your card. Please note: we do not accept ACLS or EMT certifications. BLS card must be valid through the end date of your requested elective.
- HIPAA Training Certificate: HIPAA training is required. If you have already completed HIPAA training at your home institution you can sign the HIPAA Training Certification form included in the application packet. If you have not already completed HIPAA training the Visiting Student Coordinator will register you for UW HIPAA training. Allow two weeks to receive user id and log in information. Once the training is completed, print out completion report and send or fax it.
- Application Fee: The $100 application fee is due at the time your application is submitted. If applying to more than one department send in a $100 fee for each department. Fees are non-refundable. Checks should be made out to the University of Washington. THIS FEE IS NOT DUE UNTIL YOU ARE OFFERED A CLERKSHIP.
Once all items have been received your application will then be approved and forwarded to the department(s) where the clinical elective has been requested. An approved application does not guarantee an elective. The department will then contact you within 6 weeks of the start date of the elective to confirm availability of requested rotation.
