Public Notice: Resolving Complaint Issues

Patients or family members can file a complaint with any staff member or the manager at the point of care or service, or with the Patient Relations Manager at (206) 744-8786 or (206) 744-5000. Patients can also register complaints with the Washington State Department of Health (DOH) and The Joint Commission (TJC).

Staff and caregivers are also able to use the resources below to file complaints regarding quaility of care issues.

This number is on a 24 hour-7 day a week message machine. Registered nurses will return your call Monday-Friday 8 AM to 5 PM. You may also file a complaint by printing and completing the Complaint Intake Form.  You may include any additional supporting information and mail to:
