At UW Medicine Radiology and Imaging Services, we are focusing on serving your patients and practice. If you would like to make a referral to any of our services or clinics, please follow these instructions so that we can serve you and your patient as quickly as possible.

Please complete the internal referring radiology inpatient, EPIC referral out-patient, or research requisition form.

Referring Health-Care Professionals

To refer a patient to Harborview Radiology (outpatient center), please fax patient's name, ordering clinician's full name, address, phone number, type of exam requested, clinical diagnosis, physical and medication history, insurance and authorization information to 206.744.8206.

Or, you may download and submit an Harborview radiology referral form. Upon receipt of fax, you or your patient may schedule the appointment via our centralized scheduling number at 206.744.3105.

UW Medical Center Physicians

Please complete the internal referring radiology inpatient, outpatient or research requisition form.

For the myocardial perfusion study, please complete the Myocardial Perfusion requisition form.

For Nuclear Medicine, please complete the Nuclear Medicine requisition form.

For PET exams, please complete the PET Requisition Form.
