Making an Appointment

Harborview

To make an appointment for an exam in radiology at Harborview. You will need to:

  • Get a referral form from your doctor. This needs to sent via FAX to 206.744.8206 prior to calling the scheduler.
  • Then call 206.744.3105 to talk with a scheduler. At that time, available times for the exam will be discussed with you. The preparations for the exam will also be explained to you.
  • You will receive a reminder call about your appointment from the radiology department 24 hours before your exam.
  • The scheduling office hours are 6 a.m. to 6 p.m. Mondays through Fridays.

If your health-care professional's office wants to call to make the appointment, they will need to:

  • Enter the referral via Epic or FAX the referral form for the exam to 206.744.8206 prior to calling the scheduler.
  • Then call the scheduler at 206.744.3105 to schedule an appointment and get the preparations for the exam.
  • You will receive a reminder call from the radiology department about your appointment 24 hours before your exam.
  • The scheduling office hours are 6 a.m. to 6 p.m. Mondays through Fridays.

UWMC or UWMC-Roosevelt

To make an appointment for an exam in radiology at UWMC or UWMC-Roosevelt. You will need to:

  • Obtain a referral form from your health-care professionals. FAX this form to 206.598.7690 prior to calling the scheduler.
  • Then call 206.598.7200 to talk with a scheduler. At that time, you will discuss the available times for the exam. The preparations for the exam will be explained to you.
  • You will receive a reminder call about your appointment from the radiology department 24 to 48 business hours before your exam.
  • The scheduling office hours are 8 a.m. to 5 p.m. Mondays through Fridays.

  • Fax the referral form for the exam to 206.598.7690 prior to calling the scheduler.
  • Then call the scheduler at 206.598.7200 to schedule an appointment and get the preparations for the exam.
  • You will receive a reminder call about your appointment from the radiology department 24 to 48 business hours before your exam.
