UW Medicine’s medical centers and clinics have pharmacies located on-site.

How to Refill Your Prescription

To refill a prescription, please contact the UW Medicine-affiliated pharmacy that filled the original prescription. Please have the prescription number on the bottle or packaging in hand when you call or visit. You may also request a prescription refill online at the Department of Pharmacy Services.

Prescription refills can only be processed for patients registered with UW Medical Center, Harborview Medical Center, affiliated UW Medicine Neighborhood Clinics or the Seattle Cancer Care Alliance.

During your visit, your care provider may ask what medications you are taking. You may print the Patient Medication History form (PDF) and list your medications. Take this list with you to your appointment.

UW Medical Center’s outpatient pharmacy is on the main (third) floor, while Harborview has several pharmacies throughout the medical center and at off-site clinics. Pharmacies also are located at Hall Health Center on the main UW campus and at Seattle Cancer Care Alliance.
