Refer a Patient

To refer a patient to a UW Medicine clinic, please contact MEDCON at 1-800-326-5300 or the appropriate patient care coordinator.

For a list of our specialty care clinics and specific referral information, select the appropriate facility:

• UW Medical Center
• Harborview Medical Center

Additionally, all referrals should now use the UW Medicine universal referral form.
