General Referral Information

A primary care provider must refer patients on managed care plans. Self-referrals are acceptable in certain circumstances.  When making a physician referral, please complete a UW Medicine Referral Request (PDF).  This form can be faxed to any of the locations below. Below, are specific referral instructions for each clinic within our UW Medicine system of care.

Neurosurgery Clinic at Harborview Medical Center

• Referrals can be made by contacting the Referral Coordinator at 206-744-9336 between 8:00 a.m. and 4:30 p.m. Monday through Friday. A primary care provider must refer patients on managed care plans. Self-referrals are acceptable in certain circumstances.  Clinic patients are seen at the Patricia Bracelin Steel Memorial Building location (2nd floor, 401 Broadway). 

Neurosurgery Clinic at UW Medical Center

• Appointments are scheduled Monday through Friday: Hours vary. Referrals can be made by contacting the Referral Coordinator at 206-598-9468 or 598-6489 between 8:00 a.m. and 4:30 p.m. Monday through Friday. A primary care provider must refer patients on managed care plans. Self-referrals are acceptable in certain circumstances.

UW Gamma Knife Center at Harborview Medical Center

• Typically, patients are seen in consultation and initially by a neurosurgeon or a radiotherapist. Then, our radiosurgery panel evaluates each case. After further evaluation, the appropriate method of therapy is tailored for each individual. In some cases, patients will require MRI, CT and functional MRI, angiograph and neurophysiological evaluation. Physicians can refer patients to the Gamma Knife Center at Harborview by calling 1-888-GAMMA-68 (1-888-426-6268)

Spine Surgery Clinic at Harborview Medical Center

• Referrals for the Harborview Spine Surgery Clinic should be directed to 206-744-9340.

U-Link Provides On-line Access to Medical Records

All physicians referring to Neurological Surgery will be offered the option of obtaining a secured password and directions for accessing your patient's medical records online using U-Link which provides online access to medical records.
