To schedule sleep referrals, please contact the Coordinator at 206-744-4999 or fax a UW Medicine Referral Request (Adobe PDF) to 206-744-5657.

Sleep studies cannot be ordered directly, and require prior clinical evaluation by a sleep center physician. Patients, please bring a completed Sleep Clinic questionnaire (Adobe PDF) to your initial sleep clinic appointment.

For Sleep Surgery Clinic

New patients must have a prior sleep medicine evaluation and overnight sleep study. These may be scheduled through the Sleep Disorders Center.
Outside consultation requests should include:

1. Written authorization for the evaluation, diagnostic procedures (endoscopy, nasal function studies, vigilance testing, etc.), and office therapeutic procedures (e.g., radiofrequency tissue reduction); 
2. Sleep study reports and sleep clinic notes, and any other pertinent records.

New Patients -  Complete the Sleep Surgery Questionnaire for New Patients (Adobe PDF) before your appointment with Dr. Weaver. You can mail the questionnaire or bring it with you to the appointment.

Follow Up Patients - Complete the Sleep Surgery Questionnaire for Follow - Up Patients (Adobe PDF) and bring it to each follow-up appointment.
