Oral & Maxillofacial Surgery (OMFS) Services


The OMS Clinic provides comprehensive diagnosis and treatment of a wide variety of problems relating to the mouth, teeth and facial regions. These conditions include impacted teeth, oral pathology, facial trauma, head and neck infections, maxillofacial reconstruction/implants, dentofacial deformities and temporomandibular joint disorders.

While there are no age restrictions for patients with injuries sustained from trauma or who need emergent care, the clinic’s general focus is on the adult patient. A parent or guardian who can legally give written informed consent must accompany any patient under 18 years of age.

The full services of UW Medicine are also available for other specialty services or hospitalization.

In addition to our attending physicians, the care team consists of OMS residents, general practice residents and students from the UW School of Dentistry.

About Our Staff


The OMS Clinic employs a variety of staff to support the care of patients. Some of these support staff include Dental Assistants, Registered Nurses and Patient Care Coordinators. Dental Assistants complete initial intake, take radiographs, seat the patient and assist the providers during procedures. They are also available for triage and to answer questions regarding the patient’s care. Registered Nurses assist with IV sedation procedures and provide patient education. Patient Care Coordinators help with referrals, general questions, and assist patients with scheduling surgeries and procedures.

Appointments and Referrals


Appointments for the OMS Clinic may be made by calling 206-744-3189. In most cases, the OMS Clinic will need to receive a referral from a general dentist before making an appointment for a patient. Please contact the clinic for questions regarding referral requirements.

For Referring Providers


Thank you for your interest in referring a patient to the Oral & Maxillofacial Surgery (OMS) Clinic at Harborview. These guidelines are intended to assist you in the referral process. For UW Medicine Clinics on Epic, please submit all referrals via the online Epic Referrals system. If your clinic is not on Epic, you may refer patients to our office by filling out our UW Medicine Referral Request (Adobe PDF) and faxing it to 206-744-2810. Referrals must be legible and include the following information: 

  • Referring physicians name, NPI, address, telephone and fax number 
  • Patient name, address and phone number 
  • Description of the problem and relevant clinic notes 
  • Insurance authorization for treatment must also have been obtained

Please ask the client to bring copies of recent dental x-rays, if any.
