Appointments and Referrals
Routine appointments for the Harborview Dysplasia Clinic may be made by calling the Scheduling Coordinator (number listed below).
Referring clinics should complete our UW Medicine Referral Request (Adobe PDF) and fax, along with an order regarding the patient’s latest pap smear results and their request for either a colposcopy or LEEP, as appropriate. Also required is the latest pathology report if the situation involves an abnormal pap smear.
For further guidance, see Womens Clinic Referral Guidelines (Adobe PDF).
Questions regarding referrals, managed care, and insurance coverage may also be directed to the patient care coordinator. Pre-authorization from insurance companies must be obtained prior to referrals and faxed with the referral request.
See Referrals to Harborview for general information on referring to Harborview and a listing of all referral guidelines.
