EEG & Neurophysiology Lab

The Harborview EEG & Clinical Neurophysiological Laboratory (part of the Regional Epilepsy Center and UW Medicine Department of Neurology) provides electroencephalography and related diagnostic testing services on a provider-referral basis.

Appointments and Referrals

NOTE: The University of Washington Regional Epilepsy Center Neuropsychology Lab at Harborview Medical Center is taking referrals for outpatient neuropsychological assessments.

UW Medicine Referral Request (Adobe PDF), and fax to us, along with a clinical summary with relevant information concerning the procedure(s) being requested. This Request form, along with general information on referring to Harborview and a listing of all referral guidelines can be found at Referrals to Harborview.

Documents can be faxed to the EEG/Clinical Neurophysiology Lab at 206-744-8545, attention scheduler.
