About this Clinic
Harborview’s Neurology Clinic (part of the HMC Neurosciences Clinics) sees referral patients for disorders of the central and peripheral nervous system, including headaches, seizure disorders, and neurological diseases.
Special services include patient care coordination, social worker assistance, and related neurological assessment through Neuroradiology, Neurological Surgery, Rehabilitation Medicine, and Neurology subspecialty clinics.
The full services of UW Medicine are also available for other specialty services or hospitalization.
Appointments and Referrals
New and returning appointments can be made by calling the clinic (general phone contact listed below).
Patients on managed care plans usually must be referred by a primary care provider. Self-referrals are acceptable in certain circumstances.
To make a referral, please complete a UW Medicine Referral Request (PDF) and fax to the clinic.
