Harborview provides services for some of the most complex orthopaedic injuries throughout the region. Our Orthopaedic Surgery Clinic offers numerous services including fracture care, associated soft tissue injuries, skeletal realignments and stabilizations using closed and surgical techniques.

New Patients
The Orthopaedics' New Patient Information Form (Adobe PDF) can be filled out online, then printed and either faxed or brought into our offices.
Referrals
To make a referral, please complete our UW Medicine Referral Request (Adobe PDF). Referrals should be faxed to 206-731-8661. Orthopaedic providers are available to discuss specific referral protocols and assess the appropriateness of referrals to the service.
