How do I make an appointment?

Patients are welcome to call for an appointment with one of the clinic physicians. We are happy to see you for an acute or ongoing bone or joint problem. Please be aware that many insurance programs will not pay for you to see a specialist without prior authorization, so check with your health insurance plan first. We also welcome referrals from your primary care physician or another specialist treating you. They may make a referral by phone at the number above.

What should I bring to my first appointment?

Please bring the following to your first appointment in the Bone and Joint Center:

  • Any previous medical records, including X-rays and laboratory tests.
  • If applicable, your Labor and Industry claim form, date of injury and billing information.
  • Your insurance card or current medical coupon.
