What do I need on my first visit?
Please bring the completed patient information form and your insurance card or current medical coupons.
Do I need to bring copies of my medical records?
Your doctor will send them to the Multidisciplinary Pain Center prior to your appointment. This allows an extensive record review before you visit, and ensures that you will receive the best care possible. If there are X-rays or an MRI available, you may be asked to bring them to your appointment to ensure their safe arrival.
When should I arrive for my appointment?
Plan to arrive at least 30 minutes before your appointment to check in with the front desk to complete necessary paperwork, where the staff will prepare your medical chart and process your insurance forms.
How will I be billed?
If you are used to receiving one bill from your private family doctor, our billing system may seem different to you. Instead of one invoice covering facility and physician fees, most outpatient bills will be divided up as follows:
- Your doctor's bill will come from University of Washington Physicians
- Your facility fee (which includes the use of supplies and equipment and may also include diagnostic services such as lab tests and radiology) will come from UW Medical Center
- Both UW Physicians and UW Medcial Center will bill your primary health insurance directly. You will be billed for any co-payments your insurance requires you to make.
We cannot accept co-payments at the time of your visit.
