Contents of a Grant Application

Your grant application should include some or all of these items. Universally required items are followed by an asterisks(*).

  • UW Office of Sponsored Program's Application (eGC-1)*
  • School of Medicine Classification Checklist Form*
  • School of Medicine Site Inventory Form*
  • Original letter from Department Chair to Dean in support of an application, when appropriate. Refer to section on Eligibility to Submit Grants Proposal.
  • Original letter from Dean, Chair, or Director approving request for new space or renovation of existing space, when appropriate.
  • Original Significant Financial Interest Disclosure Form signed by the P.I. and Chair, when appropriate. The sealed envelope with confidential information per GIM-10 policy should be included.  Be sure to review any requirements regarding Significant Financial Interests in advance.
  • Dean's or Vice Provost for Research's (draft or original) letter in support of an application selected through an SOM nomination process, when appropriate.
  • Letters, memos, internal budget spreadsheet, award notices, funding agency's guidelines, etc., as appropriate, to facilitate review.
  • Proposal to Funding Agency*, including: 
    • Funding sponsor's title page or UW Cover Sheet signed by P.I. 
    • Description of research plans, resources, site, and space.
    • Budget. Describe all costs and define individuals.
    • Any contract, IPA or sharing agreements, protocols, etc.
    • If the UW project will be a subcontract in another organization's proposal, then provide above items for UW project and identify lead organization's principal investigator and source of funds.
