Required Grant Contents

At minimum, the School of Medicine requires all grant applications to contain the following:
  • A complete e-GC1, detailing the grant proposal
  • An internal budget that includes a detailed list of all personnel involved in the project
  • Proposal to the funding agency that includes:
    • Sponsor title page or UW cover sheet signed by the PI
    • Scope of Work
    • Description of space
    • Description of resources 
    • Description of compliance (e.g. animals, human subjects, select agents) 
    • Budget and budget justification to be submitted to sponsor 
    • Any contract, IPA or sharing agreements, protocols, etc.
  • The following is also required, when applicable:
    • F&A Waiver form, signed by PI and Department Chair (attach to eGC1)
    • GIM-10 Form and sealed envelope, signed by disclosing individual and Department Chair (must see hard copy)
    • Cost share and matching funds addendum and approval (attach to eGC1)
    • GIM-19 waiver, signed by Lynne Chronister or other appropriate individual, confirming that OSP will accept late proposal (attach to eGC1)
If the UW is a subcontractor, grant contents should detail the UW scope of work and the e-GC1 should identify the lead organization’s PI and the source of funds (see e-GC1 question NG-3).
